Advisors: (1) Yuniar Angelia P, S.SiT, M.Kes. (2) Dwi Norma R, S.ST.One indicator to measure the quality of health services in a region is the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). The target Millennium Development Goals (MDGS) is still not achieved, where IndonesianMMR and IMR are still high. The implementation of comprehensive midwifery care is expected to improve the health standard of mother and children. So that it can decreaseMMR and IMR in Indonesia. In connection with the above fact, the purpose of this final taskis to provide comprehensive obstetric care by implementinga Continuity of Care(COC) during pregnancy, labor, postpartum, newborn, and Family Planning.Continuity of Carewas providedfor pregnant women, during labor, postpartum, newborn and Family Planning using the midwifery care management. The midwifery care given to Mrs.”S” was conducted in12 visits, which were carried out 4 times during the third trimester of pregnancy, 1 visit during labor, 4 visitsduring postpartum, 2 visitsin newbornperiodand 1 visit in Family Planning. Every visit was approached usingthe Varney mindset and documented usingSOAP note.In midwifery care, it was obtainedthat the first visit during pregnancy was physiological. However, on the third visit there was a problem of breech with KSPR score was 10. Labor was carried out by Sectio Caesarea. The baby born was 2800 grams, 49cm height, male, with 8-9 Apgar score. The baby’s weight gain for 6 days was200 gr and there were no signs of problemsduring the visit. During the postpartum, it was only found the physiological complaint such as pain in suture although sign of infection of suture wound was not found. Counseling about the benefits of exclusive breastfeeding was given due tomothers lackof confindence. Mrs.”S” preferred using condom forFamily Planning. It couldbe concluded from case studies from pregnancy to Family Planning wasphysological, even though problems during pregnancywas existed.
