Abstract
SUMMARY Hamsu, HermilindaYanita. 2017. Continuity of Care to Mrs.”L” 22 Year Old in Sri Lejaring Tyas,Amd.Keb. Midwife Practitioner Malang. Final Task. D3 Midwifery Study Program of Widyagama Husada School of Health Malang. Advisors: (1) Yuliyanik,Amd.Keb,S.KM.Biomed (2) Dwi Norma,S.ST. In Malang city in 2014 Maternal Mortality Rate (AKI) reached 135/10000 live birth and Infant Mortality Rate (AKB) was 17/1000 live birth. One of the efforts to decrease MMR and IMR is conducting comprehensive midwifery care from pregnancy until Family Planning through midwifery management approach. The purpose of provision of Continuity of Care is for early detection of complications that can be life-threatening, thus exacerbating the condition of mother and child from pregnancy until Family Planning. The method used during the case study was the Continuity of Care (COC) method, which was data collection from subjective, objective, diagnose, and perform management, and documentation in the form of SOAP note. Visits were made 12 times, 4 times during pregnancy, once during labor, 4 times in postpartum period,2 times in newborn period, and2 visits for Family Planning. The results of the care obtained during pregnancy visits until Family Planning was physiological. At the time of pregnancy, TFU was not appropriate with the age of pregnancy and the patient was given counseling about nutrition, and was encouraged to do ultrasound examination. During labor and newborn period, there were no complications. The baby born at 23:40 WIB, with 3700 grams weigth, 51 cm height, and was male. During the postpartum until the process of using condomas contraception, there were no complaints found. The conclusion of case review from pregnancy until Family Planning was physiological, and there is no gap between theory and practice. Midwives are expected to provide comprehensive care from pregnancy until Family Planning, and can apply care according to the standard of midwifery care.
