Advisors: (1) Yuliyanik,Amd.Keb., S.KM., M.Biomed (2) Yuniar Angelina P.S.siT., M.Kes Maternal Mortality rate is the number of mortality during pregnancy until postpartum with all the causes. While, infant mortality is the number of baby die before one year old with all the causes. Based on the report from the Health Departement of East Java in 2011, it was obtained that the number of maternal mortality rate (MMR) was 104.3 of 100.000 life births. The most common causes of maternal mortality rate are pre eclampsia or eclampsia and bleeding. Midwife is the spearhead of care giver for the public. One of the methods provided by midwefery is Continuity of Care (COC). Therefore the author intended to take Mrs. R 23 years old with GIIIP2002AB000 as the patient in order to provide care with COC and monitor the development of Mrs.A starting from pregnancy until Family Planning. The comprehensive midwifery care employed COC method and Varney framework, then it was documented using SOAP midwifery management. The care was conducted through 12 visits, they were 4 visits during pregnancy from 35-40 weeks gestation, 1 visit during labor, 2 visits during newborn period, 4 visits during postpartum, and 1 visit for Family Planning. The care was conducted from April to july 2017. Based on observation conducted to Mrs. “R” from pregnancy until Family Planning, it was indicated that there were no serious problems found. The problem was only back pain during pregnancy that had been handled. It could be concluded that midwifery care for Mrs. “R” was physiological. It is suggested for midwives to comprehensively conduct analysis starting from the mother’s history, family history, nutrient provision pattern and etc. References : 20 References (2009-2015) Keywords : Pregnancy, labor, postpartum, newborn, and Family Planning
