Abstract
SUMMARY Ningsih, Rianti. 2019. Comprehensive Midwifery Care to Mrs. "A" 37 years old with High Risk Pregnancy and Obesity in Pregnancy at Midwives Practice Independently Siti Nurcahyaningsing Amd.Keb Malang. Final Task. DIII. Midwifery Study Program. Widyagama Husada School of Health Sciences Malang. Advisor: 1. Senditya Indah Maya Sari, S.SIT., M.Kes. Advisor: 2. Patemah, S.SiT., M.Kes. Maternal and Infant Mortality Rate are one indicator of health development in the 2016-2018 Sustainable Development Goal’s (SDGs) in Indonesia, where MMR reaches 359 per 100,000 live births while IMR reaches 32 per 1,000 live births. The effort to reduce MMR and IMR is to increase access to quality health services for all people at every stage of life with integrated services through comprehensive interventions. The general objective of this study is to provide comprehensive midwifery care to pregnant women through the Family Planning process through the obstetric management approach. Midwifery care was done to Mrs. "A" 37 year old at Midwives Practice Independently Siti Nurcahyaningsih Amd.Keb was located in Terusan Ikan Nus No. 24, Mojolangu, Lowokwaru, Malang, East Java conducted in April-August 2019 for 13 visits: 4 visits pregnancy, 1 visit pregnancy childbirth, 4 visits puerperium, 2 visits in newborns, and 2 visits the Family Planning program. The assessment of the Poedji Rochjati Score Card (KSPR) of the mother was included in the category of High Risk Pregnancy (KRT) a score of 6, with the calculation of the initial score of pregnancy 2 and a score of 4 for ages over 35 years in this third pregnancy. The third pregnancy was also the mother's weight gain so much that it included in the category of obesity in pregnancy with a very high Body Mass Index (BMI). Based on, the results of Comprehensive Midwifery Care to Mrs. “A” there were not any pregnancy complications, pregnancy complications such as bleeding, anemia or pre-eclampsia from the first visit to the fourth visit. Gestational age ended at 39 weeks 5 days. Labor took place in the presence of a tight umbilical cord in the baby and management was immediately carried out according to the standard operational procedures (SOP). The baby boy was born spontaneously, behind the head, whimpering, redness, inactive motion, tight umbilical cord, with a weight of 3400 grams, length of 51 cm and Apgar Score 78. Postpartum care took place physiologically. Mother's Family Planning Program decided to used the contraception Intra Uterine Device (IUD) complications. It can be concluded that Comprehensive Midwifery Care Pregnancy, Childbirth, Postpartum, Newborns and Family Planning and there was not any physiologically. In the future, midwives expected to be able to maintain the quality of midwifery services and patients had the awareness to take care of their health by conducting routine checks at the Community Health Centers (PUSKESMAS) to reduce Maternal and Infant Mortality Rate in Indonesia.
