Abstract
SUMMARY F.Zentrie,Nanda, 2019. Comprehensive Midwifery Care to Ny. "U" 25 Years Old at BPM Sri Hartatik, Amd.Keb. Malang. Final Task. DIII Midwifery Study Program, Widyagama Husada school of Health Malang. Advisor: (1) Septiana Juwita, S.SiT, MPH; Advisor: (2) Yuliyanik, Amd.Keb, S.KM, M.Biomed. Pregnancy, childbirth, newborns, childbirth, and family planning are physiological conditions but in the process there is a possibility that a situation that can threaten the life of the mother and baby can even cause death. In the 2015-2019 Ministry of Health strategic plan, one of the targets to be achieved is to improve the health status and nutritional status of the community with one target, namely the Maternal Mortality Rate in 2019, down to 306 / 100,000 Live Births. In 2016, based on World Health Organization research in the world, there was a Maternal Mortality Rate of 289,000 / 100,000 Live Births, this figure did not meet the 2015 Millennium Development Goals (MDGs) target of 102 per 100,000 Live Births, the next program was the Sustainability Development Goals (SDGs) with a target in 2030 to reduce maternal mortality by 70 per 100,000 live births (Kemenkes RI, 2015). The purpose of this thesis is to provide Continuity of Care care using Varney assessment and documentation in the form of Soap Notes from pregnancy to family planning. Midwifery care to 25-year-old Mrs. "U" is based on the Poedji Rochjati 2 score card, which means this patient was at low risk because of a healthy pregnancy it was not, not fast and long pregnancy, the patient's prime age, the patient and family were very cooperative. Comprehensive Midwifery Care can be done at the Independent Practice Midwife or at the patient's home. Comprehensive Midwifery Care provided 12 visits 4 visits during the third trimester of pregnancy, 1 visit postpartum, 4, 2 times neonates visit and 1 visit family planning service. Care was given at the Sri Hartatik Independent Practice Midwife, Amd.Keb. Malang in March to August 2019 by conducting a study, diagnosis, planning, implementation, evaluation, with Subjective, Objective, Analysis, and Management (SOAP). Pregnancy care to Mrs. "U" was done at 37 weeks 2 days gestational age in the third trimester. Antenatal Care visit the patient's condition was normal. 37 weeks 5 days of gestation the patient gives birth normally entering the first stage of the dilatation phase 2 hours maximum from the opening of 6-10 cm, the second stage vaginal examination and the signs of the second is a push to push, pressure on the anus, protruding perineum, vulva and spinger ani open stage then Early Breastfeeding Initiation, the third stage injecting oxytocin and signs of detachment of the placenta, stage IV monitoring of vital signs 2 hours post partum, and monitor the condition of the baby, during the delivery process smoothly and physiologically. The baby is born spontaneously, crying strong, reddish color with a weight of 2600 grams, length 49 cm, the baby is in normal condition no visible signs of infection and emergencies.
