Abstract
SUMMARY Kurnianti. 2019.Comprehensive Midwifery Care to Mrs "A”: 41 Years Old a Very High Risk Pregnancy in Siti Nurcahyaningsih, Amd.Keb Midwife Practitioner Malang. Final Task D3 Midwifery Study Progam of Widyagama Husada School of Health Malang. Advisor :(1) Waifti Amalia, S.ST., M.Keb Advisor : (2)Yuniar Angelia P, S.siT.,M.Kes Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) is one of the main indicators of a country's health status. Based on the results of the Indonesian Health Demographic Survey (IDHS) in 2016, maternal mortality rates in Indonesia reached 305 / 100,000 live births and IMR reached 23 / 1,000 live births. According to the Sustainable Development Goals (SDGs) target in 2030, should be below 70 / 100,000 live births and IMR at least 12/1000 live births. Efforts made to improve the quality of health services for mothers and children are by conducting comprehensive midwifery care, so the midwifes can monitor and detect early complications. The purpose of this study midwife is able to provide comprehensive midwifery care from pregnancy until family planning in order to reduce MMR and IMR. Comprehensive Midwifery Care to Mrs. "A" is 41 years old GIVP2002Ab100 a very high risk pregnancy. The patient had 14 score Poedji Rochjati. The patients labor history had a very long 12 years distance pregnancy and abortion. Continuity of care was carried in June - August 2019 from pregnancy to family planning, it had been done 10 visits, consisting of, 2 antenatal care is taken from secondary data, 4 visits postpartum, 2 visits newborn and 2 visits family planning. It was conducted according to midwifery care standards using Varney management and SOAP documentation (Subjective, Objective, Assessment, Management). The results of the care provided to Mrs. “A”, the pregnancy until family planning physiologically. The labor process goes smoothly, during postpartum the mother and baby was normal, the breast milk was smooth, which was supported by the management of oxytocin massage and breast care. Mother used Intrauterine Device (IUD) family planning and there were not side effects. The conclusion continuity of care to Mrs.”A” physiologically. The writer suggests Continuity of Midwifery Care should be improved in accordance with midwifery care standards, so the complications can be detected early, to improve the level of service and to reduce maternal and infant mortality. It can be expected midwives to increase about the knowledge of pregnancy, childbirth, newborns, and family planning, which to increase the awareness of the patient, families and the people the importance of health.
