Abstract
SUMMARY Hasnah. 2019. Comprehensive Midwifery Care to Ny. "Y" Age 27 Years old Independent Practice Midwife Siti Nurcahyaningsih Amd. Keb. Final Task. DIII Midwifery Study Program, Widyagama Husada College of Health Sciences Malang. Advisor: (1) Waifti Amalia, S.ST., M.Keb, Advisor: (2) Senditya IM, S, SIT, M.Kes. Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in Indonesia are still very high compared to other ASEAN countries. The Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) reduction programs are in line with the 2016 SDG's target of 12/1000 Live Birth and Maternal Mortality Rate (IMR) of 26/1000 Live Births. The method of midwifery care in continuity of care is sustainably applied starting to the third trimester pregnant women. The purpose of this thesis is to provide Continuity of Care starting from pregnancy to family planning services in order to reduce the number of Maternal and Infant Mortality Rates. Midwifery care for 27 year old Mrs. "Y" Poedji Rochjati 6 score card which means that the patient was at high risk because the patient had a history of miscarriage. Care was given as many as 12 visits: 1 visits pregnancy, 1 visit labor, 4 visits postpartum, 2 visits newborns, then 1 visit family planning services. Care was provided at the Independent Practice of Midwife Siti Nurcahyaningsih Amd. Keb. Malang city in March until May 2019 using the midwifery management approach with the Varney mindset and documented in the Subjective, Objective, Analysis, and management forms.. Pregnancy care for Mrs. "Y" was done at 41 weeks 1 day of gestational age in the third trimester. Antenatal Care visits showed normal patient conditions. 41 weeks 2 days gestation the patient gives birth normally. The delivery process lasts when I ± 11 hours, when II ± 30 minutes, when III ± 15 minutes, and when IV ± 2 hours. During the delivery process the patient did not experience any problems and the delivery process went smoothly or physiologically. The baby was born spontaneously behind the vaginal head on May 15 2019 at 06.08 West Indonesian Time, male, weight 3800 gram, and body length 50 cm. The baby was normally seen from vital signs and found no signs of infection and emergencies. The postpartum period is normal, examination of vital signs was within normal limits, the patient's condition was good, involution and lactation were proceeding smoothly. The patient planned to used an Intrauterine Device contraceptive but temporarily used the method of intercourse interrupted. Based on, the results of continuity of care expected the patients can apply the counseling that has been given during midwifery care so that the patient and baby's condition remains good and can prevent complications.
