Abstract

SUMMARY Sari, Intan Luthfiana. 2019. Comprehensive Midwifery Care for Mrs. "E" 40 Years old with a High Risk Pregnancy at BPS Kasih Malang. Final Task. DIII Midwifery Study Program Widyagama Husada School of Health Malang. Advisor: (1) Yuniar Angelia P, S.SiT, M.Kes, Advisor : (2) Dwi Norma Retnaningrum, S.ST, M.Keb Based on, the results of the 2015 Indonesian Demographic and Health Survey (IDHS), the Maternal Mortality Rate in Indonesia reached 359 / 100,000 live births and for the Infant Mortality Rate in Indonesia reached 32/1000 live births. (RI Ministry of Health, 2015). Maternal and Infant Mortality Rate in East Java is still relatively high, based on 2018 data, Maternal Mortality Rate reaches 91.45% per 1000 births, while for Infant Mortality Rate at 13.4% per 1,000 births (East Java Health Office, 2018). The aim of Comprehensive Care Services is to improve the degree of health in Indonesia, by providing direct midwifery care through the obstetric management approach by carrying out continuity of care during pregnancy, childbirth, newborns, postnatal, and family planned. Care was given to Mrs. "E" 40 years old GIVP3003Ab000 with a high risk pregnancy. Care was carried out 12 visit. 4 visits during pregnancy, 1 visits delivery, 4 visits postpartum, 2 visits newborn and 1 visit family planning. Midwifery care conducted in March to June 2019, the implementation of care was carried out according to the standards of midwifery care with Varney's mindset documented in the Subjective, Objective, Analysis and Management (SOAP). The process of pregnancy takes place physiologically, there were complaints of swollen feet and sometimes the stomach feels stiff. Normal labor there was not any occur perinium wound and other complications indicative. The physiological puerperal period of the mother occurs complaints of heartburn and dizziness that it can be overcome by management according to theory, the normal puerperium can be seen from a good uterine involution and lactation process that did not experience problems. Physiological neonates found no problems in infants, during visits the baby's body weight increased but remained within normal limits, the mother always gave breast milk every 2 hours and the baby will be given exclusive breast milk. Mother and husband agreed to use 3-month injection contraception, because they wanted to use safe contraception which it, relatively inexpensive, easy to obtain and was not interfere with the expenditure of breast milk. The conclusion of comprehensive midwifery care to Mrs. "E" had been well achieved. The writer recommends that the Midwife be able to maintain and improve skills in providing comprehensive care to the client, so that the midwife can provide the care on target and to improve health status.
